MIDDLE SCHOOL MINISTRY MEDICAL RELEASE FORM

*For a student to attend any off-campus event, a parent/guardian must fill out and sign a UCC Medical Release Form. Release forms must be dated no more than one year prior than the event.

*Sometimes money is a legitimate obstacle for a student to attend an event. If that is the case, click the link above for our Event Scholarship Form, fill out the information, and submit it. Our team will consider and evaluate all submissions.
